A Pay Pal account is NOT REQUIRED to acquire the Premium Med ID Emergency ID Wallet Card. When asked, just select to use your Visa, Mastercard, Discover Card or American Express Card. The information and links below should answer your questions. Sorry, Telephone support is not provided for this service. Note: All recurring annual membership subscription payments for this service have been canceled! This means you can continue to use this service without having to pay additional yearly membership fees per terms of use. A one-time $5.00 setup fee is charged for new enrollee's. This program will be available as long as services are provided by third party providers.

An activation email will be sent for you to verify your account.
Activation and subscription fee required before you can print your card.

After the completion of your Med ID Card Information
